    Background

    Seizure-related homolog protein 6 (SEZ6) is a neuronal type I transmembrane protein that was originally identified as a brain-enriched, seizure-induced transcript upregulated by the convulsant pentylenetetrazol in mouse cortical neurons (1). SEZ6 is highly expressed in the developing and adult brain, where it contributes to dendritic arborization, synaptic development and plasticity, and regulation of complement activity (2-4). Genetic variants and dysregulated expression of the SEZ6 gene have been implicated in multiple neuropsychiatric and neurodegenerative disorders, including epilepsy, febrile seizures, schizophrenia, and Alzheimer’s disease (5-10). SEZ6 is a major physiological substrate of β-site amyloid precursor protein (APP) cleaving enzyme 1 (BACE1) and regulates kainate receptor trafficking and surface localization, providing a potential mechanistic link between SEZ6 and synaptic dysfunction in these conditions (3,11). Additionally, aberrant SEZ6 expression has been reported in neuroendocrine carcinomas, including medullary thyroid carcinoma, where it is being explored as a potential diagnostic marker and therapeutic target (10,12).
